If you are unsure of your benefits, CONTACT YOUR INSURANCE COMPANY DIRECTLY prior to your visit. It is important to understand your benefits. According to our vision insurance contracts with payors, contact lens services and glasses prescriptions are considered separate services and will incur additional fees.

Please provide us with your insurance information at least 3 days prior to your visit to allow our staff to verify your eligibility and benefits. When referrals from your PCP are required for a medical visit, these must be in place before we can see you.

If a medical problem is discovered during your routine eye visit, you will either be asked to return on another day for a medical evaluation or if the problem is urgent, we will use your medical insurance plan for the visit. Vision plans are for screening only and will not reimburse visits that are for medical problems or diagnostic testing.

Also, please be aware that:

• Some out of state Blue Cross plans do not have optometry coverage

• Medical visits may require insurance referrals
